Position Description:The Occupational Therapist (OT) is responsible for performing student evaluations developing and providing therapy services and documenting services in accordance with the plan of care developed for each Duties and Responsibilities:Provides skilled occupational therapy services/interventions in accordance with physician ordersRegularly reevaluates student for rehabilitation needs and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriateCollaborates with all disciplines to plan and evaluate team goals for each studentAssesses students needs plans for evaluates and modifies care to meet goals of Occupational Therapy InterventionsInstructs student/family/caregivers on the students total occupational therapy programAdvises consults and instructs members of the multidisciplinary team in specific occupational therapy modalitiesObserves and reports student progress and/or changes in student status to the physician and appropriate multidisciplinary team membersDirects OT treatment and develops occupational therapy plan of careRefers individuals to other disciplines as deemed appropriate and as approved by the physicianReports reaction to treatment and adverse signs and symptoms to the physician building/district nurse and the parents/caregiversTeaches new skills or retrains students in oncefamiliar daily activities that have become difficult or unfamiliar due to illness or injuryEstablishes equipment needs to facilitate independent functioningInstructs student and family/caregiver in the use of various types of adaptive equipment to improve function; ensures equipment is in good working orderTrains student and family/caregiver in use of prosthetic and/or orthotic devicesProvides accurate and timely documentation according to the plan of care Participates in IEP conferences or other meetings as required to ensure coordination of health services (if applicable)Prepares and submits clinical progresssummary based on the progress of goalsSupervises Occupational Therapy Assistants according to company policies/procedures and State regulationsComplies with applicable local state and federal laws and regulations regarding professional standards and practice standardsMay fabricate splints and instruct individual and/or family/caregiver in the use application and benefitsPerforms other duties as assigned/necessaryMinimum Requirements:Current licensure as an Occupational Therapist in the State of practiceOne 1 year of prior professional Occupational Therapy experience preferredParticipation and completion of Amergis Competency program when applicableCurrent CPR if applicableTB Questionnaire PPD or chest xray if applicableCurrent Health certificate (per contract or state regulation)Must meet all federal state and local requirementsSuccessful completion of new hire training as applicable to job siteUnderstand patient confidentiality and HIPAA requirementsAbility to effectively elicit/provide information to and from appropriate individuals (including but not limited to supervisors coworkers clients) via strong communication skills; proficiency in the English language is requiredComputer proficiency requiredMust be at least 18 years of ageCompliance & Ethics Expectations: Participates and successfully completes the Companys Compliance program requirements and adheres to the Code of Conduct Company policies and applicable federal and state requirementsSets an example for other employees regarding how the Companys Code of Conduct and Compliance Program is applied and observed every day when dealing with customers business operations or other team membersReports potential violations of Company policy Code of Conduct and/or applicable laws and regulations to the Company hotline through the chain of command to the Compliance and Ethics Department or through other channels made available by the Company for reporting potential violationsPromotes an environment in which other employees are encouraged to report potential violations As appropriate provides input and suggestions regarding areas in which policies procedures workflows and/or controls can be improved to enhance compliance
