The PGY1 pharmacist residency is a structured 1year postgraduate educational and training experience that fosters the development of advanced knowledge and skill in the delivery of pharmaceutical services within a community hospital setting.
Major Responsibilities:
The following key areas (Clinical Operations/Distribution Financial Safety and Regulatory Research and Education) are performed by the Pharmacy Resident (PGY1 through the completion of rotations under the supervision of preceptors in each specialty which includes but is not limited to anticoagulation antimicrobial stewardship critical care family medicine internal medicine oncology medication safety practice management transitions of care and staffing and operations.
Clinical Performance:
Provides medication management services for all assigned patients in a collaborative manner that consist of the following:
- Review patient medication history educate patients on drug therapy establish desired outcomes develop/modify therapeutic plans determine alternatives for achieving outcomes develop drug monitoring plans monitor patient outcomes document medication related issues in electronic health record interpret laboratory data review patient medication profiles monitor pharmacokinetics of drug therapy monitoring for potential adverse drug drug druglab drugdisease state and drug food interactions.
- Maintains and updates a patient profile with demographics (e.g. height and weight) diagnosis allergies and current medications.
- Monitors drug therapy to evaluate appropriateness of use dose dosage form regimen route therapeutic duplication and drug interaction.
- Provides drug information to health care professionals and to patients.
- Documents clinical interventions.
- Counsels patients on discharge medications as appropriate.
- Performs clinical review and verification of medication orders.
- Performs medication order entry when needed.
- Discusses medication order clarifications with the prescriber documents interventions and communicates medication order changes as needed.
- Facilitates medication discharge planning and smooth transition to alternate sites of care and access to an ambulatory pharmacy.
Operations/Distribution Performance:
- Reviews and/or verifies medication orders for accuracy and appropriateness.
- Ensures the timely and accurate dispensing of medications from the pharmacy area.
- Ensures proper technique and accurate preparation of all pharmaceutical products including oral nonsterile and sterile compounding chemotherapeutic hazardous and investigational agents.
- Responsible for the accurate filling and dispensing of medications from the pharmacy area.
- Ensures regulatory compliance of controlled substance distribution and monitoring.
- Supervises work of pharmacy technicians.
Financial Performance:
- Promotes the use of formulary by converting nonformulary orders to formulary when possible according to P&T protocol. Coordinates procurement of nonformulary when necessary.
- Conducts targeted drug program and drug usage evaluations as assigned and reports results to the Pharmacy and Therapeutics Committee.
- Identifies and acts upon cost avoidance in drug therapy by promoting cost effective regimens and by reducing waste.
- Facilitates cost saving programs such as IV to PO and therapeutic substitution.
- Manages medication inventory expiring products and recalls.
Safety and Regulatory Performance:
- Detects monitors documents and reports adverse drug reactions and medication errors.
- Utilizes drug use reports to identify adverse drug events.
- Participates in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan.
Research and Education Performance:
- Coprecepts Intermediate Pharmacy Practice Experience (IPPE) rotation students and Advanced Pharmacy Practice Experience (APPE) rotation students consistent with academic credentials and clinical experience.
- Participates in continuing education programs meetings training programs and related activities as assigned.
- With guidance from the residency program director and residency preceptors designs and completes a research project suitable for publication and presentation at the American Society of Health Systems Pharmacists (ASHP) Midyear Meeting and the Eastern States Residency Conference.
- Attends all assigned conferences potentially including ASHP Midyear Clinical Meeting MSHP Annual Meeting and the Eastern States Residency Conference.
- Prepares and provides effective inservice presentations on a variety of disease state medication and practicerelated topics to pharmacy staff other hospital staff members pharmacy students patients and caregivers as determined by the residency program director.
Resident Specific Responsibilities:
- Abides by ASHP residency accreditation standards and achieves residency goals and objectives as outlined in the Residency Manual Requirements for Completion of Residency.
- Maintains a professional portfolio of work completed throughout the residency year.
- The Resident must participate in the recruitment efforts of the Residency program.
Standard Staffing Level Responsibilities:

- Attends variety of meetings conferences seminars as required or directed.

Position Qualifications:
License/Certification/Education:
Required:
- Doctor of Pharmacy graduate candidate from an ACPEaccredited school of pharmacy (or one in process of pursuing accreditation) or have a Foreign Pharmacy Graduate Equivalency Committee (FPGEC) certificate from the National Association of Boards of Pharmacy
- Licensed Pharmacist in the Commonwealth of Massachusetts
OR
- Registered Pharmacy Intern in the Commonwealth of Massachusetts and must obtain aMassachusetts pharmacist licensure by October 31st.
Experience/Skills:
Required:
- Effective oral and written communication skills.
- Ability to work independently as well as working with others in a team environment.
- Proficient with office systems and computers including Microsoft Word Excel and PowerPoint.
