Located in southeast Charlotte near SouthPark and Cotswold Atrium Health Sardis Oaks is an awardwinning nursing and rehabilitation facility with 119 beds. We provide short term rehab long term care palliative care and hospice care and are committed to enhancing the quality of life and wellbeing of all our residents. LPN teammates at this facility will assist with medication administration assessments wound care bedside care lab draws and IV Oaks has onsite doctors or nurse practitioners five days a week. Recently Sardis Oaks was recognized by Centers for Medicare and Medicaid Services for providing 5 star care and was named one of U.S. News & World Reports Best Nursing Homes for Shortterm Rehabilitation.

Job Summary

Coordinates all processes required for accurate and timely completion of the Minimum Data SetResident Assessment Instrument/Inpatient Rehabilitation FacilityPatient Assessment Instrument. Participates in the residents/patients comprehensive plan of care. Serves as the leader and /or member of the interdisciplinary team. Ensures all regulatory compliance and industry standards are met. Maintains clinical and professional competency as appropriate to the age culture development stages and special needs of the residents/patients served. Maintains current knowledge of changes in guideline impacting the MDSRAI/IRFPAI process.



Essential Functions

Physical Requirements
Work requires walking standing sitting lifting reaching stooping bending pushing and pulling. Must be able to lift and support the weight of 35 pounds in handling residents/patients medical equipment and supplies. Must speak English in good understandable terms. Intact sense of sight hearing smell touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in resident/patient and/or unit conditions.



Education Experience and Certifications

Graduate from an accredited School of Nursing. Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency meet the licensure requirements in your home state; or for NonNational License Compact States current RN License or temporary license as a Registered Nurse Petitioner required in the state where the RN works. BSN preferred. BLS required per policy guidelines. Resident Assessment CoordinatorCertified strongly preferred. 1 year experience with MDS/IRFPAI and/or prospective payment system (PPS). Additional education training certifications or experience may be required within the department by the nurse leader.
