Hours:Exempt Min. 40 hours per week

Schedule: 7 days per pay period with a rotation of 7 days working and then 7 days off.

• Monday Friday: 4:30 pm 8:00 am
• Friday at 4:30pm till Monday Morning at 8:00am.

What Youll Do in This Role:

The Huron Valley PACE Extended Care Nurse provides afterhours care and weekend coverage for participants working 7 days per pay period on a rotating schedule 7 days on 7 days off). Responsibilities include taking nursing calls assessing needs for emergent visits 4:30 p.m. 8:00 a.m. and working scheduled weekend shifts 8:00 a.m. 4:30 p.m. Saturday and Sunday). Oncall hours are from 4:30 p.m. Friday to 8:00 a.m. Monday. The nurse collaborates with the Huron Valley PACE Interdisciplinary Team to deliver integrative care to frail elders focusing on enhancing functional capacity promoting autonomy and ensuring coordinated care.

Primary Responsibilities:

• Provide advanced nursing care as per clinical treatment plans and conduct participant visits based on assessments.
• Assess nursing needs through phone or inperson evaluations and address participant concerns documenting actions taken.
• Perform diagnostic preventive and rehabilitative nursing procedures ensuring safety and proper care and communicate with the clinical team.
• Monitor participants conditions and report changes to the clinical team as needed.
• Educate and counsel participants and families on care needs and procedures based on the participants condition.
• Maintain effective communication and ensure compliance with the participants Plan of Care.
• Maintain accurate timely care progress records and submit documentation per organizational standards.
• Follow organizational policies related to service delivery documentation and care coordination ensuring compliance during supervisory visits.
• Actively participate in quality improvement initiatives.
• Effectively triage nursing calls for participants.
• Collaborate as a key member of the Interdisciplinary Team attending meetings and contributing to care planning.
• Perform other related duties as required adapting to organizational needs.

What Youll Need:

• Degree in nursing; must be licensed to practice as a registered Nurse (RN) in the state of Michigan.
• Must have a minimum of 1 year of nursing experience.
• Previous Home Health or Hospice experience preferred.
