Position: Physical Therapist Home Health

Position Summary:
Providence Health & Services is seeking a dedicated and experienced Physical Therapist (PT) to provide skilled home health services throughout West Portland and surrounding areas including Newberg Dundee and Willamina. The ideal candidate will have strong clinical judgment excellent time management and experience in Skilled Home Health.
Key Responsibilities:
- Deliver highquality physical therapy care to patients in their homes
- Manage a caseload of up to 6 patients per day (standard: 30 productivity points/day)
- Conduct patient assessments develop care plans and perform treatments
- Document visits using Epic Remote Client and Rover App including OASIS documentation
- Communicate with interdisciplinary care team regarding patient progress
- Travel between patient homes within the assigned territory (transportation required)
Requirements:
- Minimum 1 year of recent Skilled Home Health experience
- Active Physical Therapist license in the state of Oregon (pending license accepted)
- BCLS certification required
- OASIS experience required
- Reliable transportation for home visits
- Comfortable driving up to 60 miles/day (max 60 90 minutes roundtrip)
Preferred Skills & Experience:
- Experience with billing ordering supplies and scheduling appointments
- Proficient in lifting and patient transfers
- Wound care certification and PTA supervision experience preferred
Additional Information:
- Mileage Reimbursement: Standard IRS rate
- Weekend Requirement: No
- FirstTime Travelers: Accepted
- Dress Code: Business casual or scrubs (any color); jeans not permitted
