Pay Rate: $73/hr (W2 $78/hr 1099
Job Summary:
Providence Portland Home Health East is seeking a dedicated and experienced Occupational Therapist (OT) to provide highquality home health services to patients in East Portland and surrounding areas. The OT will deliver care within a 1:1 patient ratio visiting up to 6 patients per day with productivity targets of 30 points daily. Travelers must have their own transportation and while occasional daily driving can reach up to a 60mile radius 60 90 minutes total drive time) the system is structured to group client visits efficiently to minimize windshield time.
Key Responsibilities:
- Deliver skilled OT services in patients homes across East Portland and nearby regions.
- Conduct assessments and develop treatment plans focused on patient independence and rehabilitation.
- Maintain accurate and timely documentation using EPIC and Rover systems.
- Collaborate with interdisciplinary teams to support patients holistic care.
- Navigate a manageable driving radius with support from efficient client scheduling.
Requirements:
- Experience: Minimum 1 year of skilled home health OT experience required.
- Certifications: Current BCLS certification.
- Licensure: Active Oregon OT license (pending license accepted).
- Charting Experience: Proficiency with OASIS required.
- Transportation: Must have own vehicle for patient visits.
- Vaccination: Completion of primary COVID vaccination series 1 booster required.
Pay Details:
- Pay Rate: $73/hr (W2 $78/hr 1099
- Overtime Pay: 1.3x
- Mileage Reimbursement: IRS Standard
- Parking: Free onsite
Additional Information:
- Attire: Business casual (scrubs allowed no specific color; no jeans)
- Weekend Requirement: None
- Local Candidates: Accepted (within 50 75 mile radius same rate)
- RTO: Confirm with hiring manager
