Job Description

Responsibilities:

* Provides case management to individuals to help them understand and overcome personal or social issues *Coordinate counseling and therapeutic services with other professionals such as doctors nurses psychiatrists and social workers

* Conduct consistent followup interviews with clients to determine if their needs are met

*Maintain appropriate and timely documentation progress notes and screening/assessment tools *Responsible for caseload updates data collection and submission of activities reports

*Ensure that clients are housing ready

*Prepare housing packages

*Ensure that clients are keeping all outside appointments

*Minimum associate degree *Multiculturally competent and strong interpersonal skills *Knowledgeable of all city and state & federal entitlements benefits * Knowledgeable of various community resources and partners

$55000 yr.
