Summary The 6month social work fellowship provides an opportunity for fellows to gain handson experience and develop essential skills in social work and public health. Under the supervision of experienced social workers you will provide direct services to clients participate in case management and contribute to community outreach initiatives in the Indianapolis Metropolitan area.
Program Highlights The Focus Lead and Collaboration Community Outreach Fellow will provides a learning environment onsite a neighborhood community center for recent graduate Social Workers an Public Health workers to work alongside processionals and external community partners to address the complex needs of individuals and families in the Northwest and Northcentral Indianapolis area including part of Pike and Washington township residents.
Upon Completion of Program
- Fellows will gain experience in handson outreach activities within the community including being part of a mobile case management unit that provides services to Northwest and Northcentral Indianapolis areas including parts of Pike and Washington Township area residents.
- Fellows will gain experience in conducting intake assessments developing individualized care plans and providing ongoing case management support to clients.
- Fellow will gain experience developing a sustainability plan in order to capture the programs short and longterm success and outcomes.
- Fellows will gain leadership skills through opportunities to lead community initiatives coordinate outreach activities and collaborate with internal and external teams.
- Fellows will receive mentorship and supervision from experienced social workers and public health professionals to enhance their skills knowledge and competencies in the field.
Responsibilities
- Assist in conducting intake interviews and assessments to evaluate clients needs strengths and challenges.
- Collaborate with licensed social workers and other members of the interdisciplinary team to develop comprehensive treatment plans tailored to clients goals and objectives.
- Provide direct support and counseling to clients including individual and group sessions to address emotional behavioral and social issues.
- Manage the daytoday operations of the mobile program which encompasses a range of activities such as coordinating job fairs developing presentations and scheduling events etc.
- Assist clients in accessing community resources such as housing healthcare employment and educational opportunities by providing referrals and coordinating services.
- Advocate for clients rights and access to services by engaging with relevant agencies organizations and community stakeholders.
- Maintain accurate and uptodate documentation of client interactions progress notes and service plans in accordance with agency policies and procedures.
- Participate in case conferences team meetings and supervision sessions to review cases discuss challenges and receive feedback on performance.
- Adhere to ethical standards and professional guidelines.
- Engage in ongoing professional development activities including training sessions workshops and seminars to enhance knowledge and skills in the field of social work.
- Perform other duties and responsibilities as assigned by supervisor.
Qualifications
- Bachelor of Social Work or Public Health recent graduates; organization would consider individuals seeking a bachelors degree in social work or public health field.
- Knowledge of social work theories principles and practices related to human behavior development and social systems is a plus.
- Strong interpersonal skills with the ability to establish rapport and build trust with diverse populations.
- Excellent communication skills both verbal and written with the ability to effectively document client interactions and communicate with colleagues and stakeholders.
- Ability to work collaboratively as part of a multidisciplinary team and independently with minimal supervision.
- Empathy compassion and a genuine desire to help individuals and families overcome challenges and improve their quality of life.
- Proficiency in Microsoft Office Suite.
- Reliable transportation and willingness to travel to mobile location/client homes community agencies and other locations as needed.
- Fellow shifts: MF 11am to 8pm; Not yet confirmed: Potential shift need 8am to 5pm
- Paid Fellowship: $17.00 to $20.00 based on experience. Fellow will have an opportunity after successful completion of program to be considered for staff position at neighborhood center.
