Licensed Physical Therapist Home Health (PT) $105000$115000

Essential Responsibilities:
- Perform comprehensive physical therapy evaluations and functional assessments
- Create and implement evidencebased treatment plans aligned with patient goals
- Deliver handson therapeutic interventions including manual therapy and exercise prescription
- Provide patient and caregiver education on rehabilitation techniques and injury prevention
- Maintain detailed documentation of treatment plans and progress notes
- Coordinate care with interdisciplinary healthcare team members
- Ensure compliance with Medicare guidelines and insurance requirements
Required Qualifications:
- Current Physical Therapy license in good standing
- Graduate of an accredited DPT/PT program
- Minimum 9 months of clinical experience
- Valid drivers license and reliable transportation
- Strong clinical reasoning and problemsolving abilities
- Excellent communication and interpersonal skills
- Experience with electronic medical records (EMR)
