Job Description

Description

Provides medication management services in coordination with other health care professionals to deliver safe appropriate and costeffective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens medication therapy management medication order verification/entry medication distribution medication therapy monitoring and consultation documentation of medication therapy plans and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocoldriven practice as appropriate. Additionally pharmacists are active members in quality improvement research activities and education as described in the job specific competencies.



Qualifications
  • Graduate of an ACPEaccredited Doctor of Pharmacy (Pharm.D. program or Bachelor of Science (B.S. with a minimum of 2 years relevant experience.
  • Other advanced degrees (e.g. Masters Ph.D. and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor preferred.
  • Preferred Qualifications:
    • Completion of pharmacy residency and specialization (e.g. PGY2 and respective BPS certification) if applicable.
    • Evidence of ongoing scholarly pursuits including research/quality improvement publications or scientific presentations.
    • Relevant applicable pharmacy experience working in a progressive and collaborative practice environment.
    • Experience and/or interest in teaching and/or precepting.
  • Commitment to excellence accuracy attention to detail and teamwork required. Ability to communicate effectively work collaboratively with others organizes time well practice with an agile mindset solve problems and work independently with minimal supervision required.

License or Certification

  • Current pharmacist license by the WI Board of Pharmacy or eligible to obtain within a time frame compliant with the sites applicable employee licensure policy. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
