Job Description:
We are seeking a highly motivated and experienced Home Health LPN to join our team in Canonsburg PA. In this role you will work collaboratively with a multidisciplinary team to provide compassionate and comprehensive care to patients in their homes.
Responsibilities:
Conduct initial and ongoing assessments of patients health status
Develop and implement individualized care plans based on patient needs
Administer medications and treatments as prescribed by the physician
Monitor and document patient progress and report any changes to the physician
Educate patients and their families on proper care techniques and disease management
Collaborate with other healthcare professionals to provide coordinated and holistic care
Maintain accurate and uptodate patient records
Adhere to all state and federal regulations and company policies
Qualifications:
Current LPN license in the state of Pennsylvania
Minimum of 1 year of experience in home health or a related field
Strong communication and interpersonal skills
Ability to work independently and in a team environment
Excellent time management and organizational skills
Compassionate and empathetic attitude towards patients
Basic computer skills and proficiency in electronic medical records
Valid drivers license and reliable transportation
