Education:

  • Masters Degree in Social Work (required)

Certifications/Experience:

  • Three years experience in Acute Medical Social Work (Preferred)
  • Prior experience in perinatal services (Preferred)
  • Indiana Licensed Social Worker (Required)

The perinatal social worker provides social care to individuals babies and families experiencing challenges before during and after pregnancy including through the first year of a babys life. Responsibilities include helping families who experience highrisk pregnancies premature or sick newborns family conflicts legal issues parents with mental health needs and families in poverty. Duties also include helping parents manage the psychosocial conditions they experience during pregnancy and after birth. Communication problemsolving and timemanagement skills are essential.

General Responsibilities:

The duties of a perinatal social worker include but are not limited to:

  • Assists patients during the perinatal period by addressing social physical and mental health concerns
  • Maintains active list of community resources and support groups for patient referrals
  • Assesses unique needs of high risk antepartum patients
  • Helps families process complex information related to pregnancy and neonatal conditions/diagnosis.
  • Works with adoption and/or surrogacy agency representatives. Collects and reviews all associated legal paperwork for completeness. Informs nursing staff of the upcoming cases and the individualized birth plan for each.
  • Help families cope with grief in cases of perinatal loss like stillbirth miscarriage or neonatal death.
  • Assists families who have a parent or child with developmental or intellectual disabilities with communal support and resources.
  • Addresses perinatal issues that affect the mother the infant and their families.
  • Addresses any mandatory reporting and collaborates with and is liaison between CPS and care team members.
  • Addresses symptoms of perinatal anxiety and depression. Follows up with patients who score high risk on the Edinburgh discharge screening.
  • Provides individualcentered culturally competent care practices.
  • Educates doctors nurses and other healthcare professionals on best practices for individualcentered care.
  • Enhances healthcare equity which is related to the social determinants of health.
  • Facilitates the discharge planning in collaboration with the Interdisciplinary team by coordinating and communicating with the unit based RN CM members of the care continuum patient family and physician.
  • Provides information on community resources to assist with financial coverage in coordination with Hospital Financial Counselors as available
  • Maintains contact with Post Acute Care Providers and Community Resources Agencies used for referrals.
  • Oversees hearing screen program and completes necessary state reporting and paperwork.
  • Performs follow up discharge calls to patients
