As a valued member of our collaborative and supportive team you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward. The teams at Easterseals PORT Health care directly for our clients in the community including visits to clients living in their own homes.
What Youll Do
As a Qualified Professional in the Community Support Team (CST) you will provide services to individuals who have a mental health or substance use disorder and meet the two life domains. As a Qualified Professional you will provide support services by participating in the provision of structured rehabilitative interventions to individuals; such as housing and tenancy money management daily activities and personal wellness.

This fulltime position offers a Monday Friday flexible nontraditional schedule and will serve the Surry and Yadkin Counties.
