Job Description

Company Information: A healthcare company with corporate office located in Indianapolis Metro area serving more than 100 million people at every stage of health.


Location: This position is primarily remote. Candidates must have an appropriate home workspace and be available for occasional inperson team meetings.


Job Summary: The LTSS Dual Liaison please a critical role in coordinating and managing care for individuals eligible for both Medicare and Medicaid. This position involves acting as a key intermediary between healthcare providers social services and patients ensuring comprehensive and integrated care. The LTSS Dual Liaison works to improve health outcomes and quality of life for individuals requiring longterm service and supports.


Job Duties:

Develop and implement personalized care plans for dualeligible beneficiaries.

Coordinate services among healthcare providers social services and community resources.

Facilitate effective communication between all parties involved in the patients care.

Advocate for the needs and preferences of patients ensuring they receive appropriate and timely care.

Assist patients and their families in navigating the complexities of Medicare and Medicaid.

Work collaboratively with healthcare providers social workers case managers and other stakeholders to deliver holistic care.

Facilitate regular interdisciplinary team meetings to discuss patient progress and adjust care plans as needed.

Identify and connect patients with appropriate longterm services and support including home and communitybased services (HCBS).

Monitor and evaluate the effectiveness of service provided making adjustments as necessary.

Maintain accurate and thorough documentation of patient interactions care plans and service coordination.

Ensure all activities comply with relevant Medicare and Medicaid regulations and organizational policies.

Provide education and training to patients families and care providers on navigating the healthcare system and accessing services.

Stay current with industry trends best practices and regulatory changes.


Job Qualifications:

Must have a BA/BS with a minimum of two years working with social work agency LTSS healthcare and/or aged and disability industry experience.

Experience in care coordination case management or related role within the healthcare or social service field.

Experience working with dualeligible populations and deep understanding of Medicare and Medicaid programs.
Hours: M F 8am to 5pm EST; Starting Salary $45 000 to $60000 (based on experience) Health Insurance and Benefits offered.
