As a Director of Clinical Strategy and Initiativesfor Home Care youll play a pivotal role in shaping the future of healthcare delivery all while making a real difference in the lives of the patients we serve.

This is a fulltime positionoffering $115000 $125000per year Monday through Friday 8am 5pm). And youll be part of an exceptional team thats passionate about providing the Right Care at the Right Time in the Right Place.

What Youll Be Doing:

  • Strategic Leadership:Lead innovative clinical initiatives and set priorities to enhance patient outcomes and care delivery across all home care services.
  • Program Development & Oversight:Design implement and evaluate clinical programs that improve care quality safety and effectiveness.
  • Quality Improvement:Drive initiatives to improve clinical outcomes patient satisfaction and operational efficiency using data analytics and performance metrics.
  • Collaboration:Work alongside a passionate multidisciplinary team to provide the best care possible while meeting state and federal regulations.
  • Clinical Team Support:Mentor and guide healthcare professionals to achieve excellence through ongoing training support and fostering a culture of continuous learning.

What Were Looking For:

  • Education:Bachelors degree in Nursing (BSN) required; Masters in Nursing Healthcare Administration or related field preferred.
  • Experience:810 years in clinical leadership roles in home care with proven success in ValueBased Purchasing (VBP) program management.
  • Licensure:Current Registered Nurse (RN) license in New York State.
  • Skills:Strong leadership excellent communication and proficiency in data analytics and healthcare technologies.
