Home Health Physical Therapist Senior Rehabilitation Specialist

- Provide expert education to patients and caregivers on rehabilitation techniques
- Document patient progress and maintain detailed electronic health records
- Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
- Ensure compliance with Medicare guidelines and healthcare regulations
Required Qualifications:
- Masters degree in Physical Therapy from an accredited program
- Current state Physical Therapy license in good standing
- Minimum 1 year of home health physical therapy experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong communication and interpersonal skills
- Proficiency in electronic documentation systems
