Essential Job Duties:
- Program Development and Maintenance
- Patients identify and request program services.
- Maintains excellent relationship with health staff physicians and external groups as assessed through evaluation tool.
- Coordinates planning with multiple departments and health care providers.
- Enhances program through research utilization.
- Sets and meets outcome goals in collaboration with VP
- Develops relationship with healthcare staff physicians and external groups
- Develops Cancer resource packet using evidencebased practice
- Successfully accomplished goals and action plans related to operation/business plan
- Assist VP in accomplishing goals and action plans related to operations/business plan
- Promotes staff accountability for their practice
- Helps community resources collaborate with patient navigation and education
- Responds to 95100 of patient calls within 24 hours on weekdays
- Initiates communication with patients upon learning they have a suspicious or positive finding on diagnostic studies
- Serves as a liaison between the patient medical staff and services
- Plans and implements a collaborative program with referral services to ensure a seamless delivery of care
- Provides education and consultation within scope of care
- Utilizes community resources for treatment and support for overall health of patients served
- Participates in interdisciplinary cancer clinic and tumor boards
- Makes contact to informs patient of their initial appointment with an oncologist and orients the patient to the navigation role and expectations prior to their initial visit
- Seeks working relationships with other navigators for idea sharing
- Participates in appropriate community groups and committees to gain knowledge and enhance program
- Attains >20 approved CEUs in oncology or applicable areas per year
- Competent to explain all the diagnostic and therapeutic procedures performed associated with navigated oncology population
- Demonstrates a thorough knowledge about community resources for treatment and support for general health related to treatment and disease for the navigated population.
- Reporting and Record Keeping
- Creates new reporting tools
- Documentation completed in 100 of records
- Tracks patients as they move through the healthcare system
- Provides timely and complete documentation for accurate patient care
- Tracks interventions and outcomes.
- Customer Service/Marketing
- Customer Satisfaction Evaluation > 95
- Keeps immediate supervisor and team members fully informed concerning work progress
- Proactively works to resolve issues to improve quality and efficiency of department
- Consistently displays professionalism in dealing with patients and peers
- Embraces new ideas and procedures that will improve departmental performance
- Provides or collaborates on community education events
- Initiates opportunity or seeks collaboration with other departments
- Aids in a clear courteous professional and flexible manner both orally and in writing

- Creates annual marketing plan in collaboration with Marketing Department

Basic Qualifications:
Associates Degree in Nursing
Current licensure as a registered nurse from the Virginia State Board of Nursing
BCLS
At least three years of clinical practice in the healthcare setting oncology preferred. Must possess the ability to communicate orally and in writing work effectively in a team environment have excellent interpersonal skills and possess knowledge of community resources hospital processes structure and function. Oncology background preferred and/or disease specific care coordination.
Preferred Qualifications:
Bachelors Degree in Nursing
Navigator Experience
Prior experience in Urology or related specialty practice a plus.
