Job Description

Summary

Under general direction from the Director of Pharmacy Services responsible for the development monitoring and improvement of medication policies procedures standards of care and best practices for medication safety.

Education

Graduate of an accredited College of Pharmacy. Graduate degree in Pharmacy or Business preferred.

Certification Registration & Licensure

Required Upon Hire: Licensed to practice pharmacy in the State of New Hampshire or reciprocity pending within 90 days of hire.

Board Certified Pharmacotherapy Specialist (BCPS) is desirable.

Experience

Minimum of five years of progressively responsible experience in an acute care hospital pharmacy. Pharmacy residency preferred.

Responsibilities

  • Identifies best practices for medication safety.

  • Analyzes facility reporting systems to determine their effectiveness in terms of adverse drug event

    capture timeliness of reporting and usefulness of information captured.

  • Analyzes controlled substance distribution system. Reviews processes for security and best practices.

  • Monitors compliance with controlled substance security standards throughout the enterprise.

  • Participates in noncompliance investigative interdisciplinary focus teams to analyze data develop a failure mode and effects analysis and makes recommendation for change or disciplinary follow up.

  • Formulates educational sessions for nurses pharmacists physicians and allied health professionals

    based on reports in the literature responses to corrective action plans and recommendations from

    committee activities.

  • Reviews medication variance reporting system on a daily basis.

  • Performs a system wide analysis of medication errors and adverse drug events based on data reported.

  • Participates in facility based root cause analyses.

  • Participate in various safety committees including Pharmacy and Therapeutics Medication Safety Patient Safety Safety and Hazardous handling committee.

  • Assists in Quality Assurance and Patient safety by reviewing reported outcomes data to NH State CMS

    or other agencies.

  • Assists in formulary reviews for patient safety by evaluating new or drug classes for adverse effects special handling look alike sound alike names and other characteristics relating to medication safety.

  • Performs duties of a pharmacist including order entry drug information pharmacokinetic dosing

    monitoring and documentation.

  • Performs other projects or duties as required or assigned.
