Role Overview

Key Responsibilities
- Conduct comprehensive physical assessments and evaluate patients functional capabilities
- Design and implement individualized treatment plans aligned with patient goals
- Perform therapeutic exercises and manual therapy techniques
- Provide patient and family education on rehabilitation processes and injury prevention
- Document patient progress and maintain detailed clinical records
- Collaborate with interdisciplinary healthcare teams
- Ensure compliance with Medicare/Medicaid guidelines and agency policies
- Participate in care planning and case management meetings
Required Qualifications
- Masters degree in Physical Therapy from an accredited program
- Current state Physical Therapy license
- Minimum 1 year of home health experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong clinical documentation skills
- Excellent interpersonal and communication abilities
