General Job Description:
The Nurse Practitioner is responsible for promoting health and safety of their patients by providing quality medical care through proactive treatment and diagnoses. Their duties include addressing patients health concerns conducting comprehensive health assessments physical exams and screening for chronic medical conditions.
Qualifications:
EDUCATION
Graduation of an accredited school of nursing.
Successful completion of an accredited Nurse Practitioner program.
CERTIFICATE/LICENSE
Georgia State Registered Advance Nurse Practitioner License
ANA certification
CPR/BLS

EXPERIENCE
Minimum of twoyears clinical experience preferred.
KNOWLEDGE SKILLS & ABILITIES
Knowledge of professional nursing theory practice and medical care to give and evaluate patient care.
Knowledge of medical equipment and instruments.
Knowledge of common safety hazards and precautions to establish a safe work environment.
Ability to assess medical conditions and interpret findings.
Establishing and maintaining effective working relationships with patients medical staff and the public.
Ability to react calmly and effectively in emergency situations.
Ability to communicate clearly with staff and patients.
Ability to make patients comfortable and feel welcome.
ESSENTIAL DUTIES & RESPONSIBILITIES (include by not limited to)
Obtains complete medical history and physical data on patients.
Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.
Performs a complete physical exam and records findings within 48 hours or exam. Collaborates with physicians in managing acute and longterm medical needs of patients and assists physician in handling medical and surgical emergencies.
Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
Administers medications and injections according to physician directions and performs other clinical functions as needed. Orders appropriate laboratory and diagnostic procedures.
Provides patient education. Interview and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs. Instructs patient and family regarding medications and treatment instructions.
Maintains and reviews patient records charts and other pertinent information. Post tests and examination results. Completes detailed patient charts within 48 hours.
Triages patient telephone calls and provides consultation. Provides monitoring and continuity of care between physician visits.
Attends required meetings and participated in committees as requested.
Participates in professional development activities and maintains professional affiliations.
Maintains patient confidentiality.
Performs related work as required.
Adheres to PCMH ACO HRSA and any other associated organization requirements.
Practice and adhere to the Code of Conduct philosophy and Mission Statement.
Provider shall also be oncall for his area of specialty and shall share in the established Provider OnCall Schedule to include patient chart followup documentation.
CAQH must be attested to and kept current.
DOT certification required within 30 days of employment.
Labs reviewed within 48 hours of receipt.
Task box assignments reviewed daily and completed within 48 hours. Prioritizing emergent cases.
Charts completed and closed within 48 hours.
Drug screens for Schedule 3 or any narcotics and register prescriptions on the PDMP (Prescription Drug Monitoring Program)
Promotes TMFC Pharmacy for prescription fills and refills when possible
Referrals to Specialist when procedure cannot be done inhouse.
Other duties as assigned.
Physical Demands:
Requires full range of body motion including manual and finger dexterity and handeye coordination.
Requires standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 25 pounds.
Requires corrected vision and hearing to normal range. May require working under occasional stressful conditions and/or irregular hours.
TYPICAL WORKING CONDITIONS:
Infrequent exposure to communicable diseases toxic substances medical preparations and other conditions common to a clinic environment

TMFC provides healthcare services as a Federally Qualified Health Center (FQHC). FQHCs are communitybased health care providers that receive funds from the HRSA Health Center Program to provide primary care services in underserved areas. They must meet a stringent set of requirements including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes patients.
The defining legislation for Federally Qualified Health Centers (under the Consolidated Health Center Program) is Section 1905(l)2(B) of the Social Security Act. TMFC has also been awarded recognition by the National Committee for Quality Assurance (NCQA) PatientCentered Medical Home Program. NCQAs PCMH Recognition program is the most widely adopted PCMH evaluation program in the country.
TriMed Family Care is a subrecipient of a Georgia Family Planning System (GFPS) grant and is deemed a family planning service site. GFPS ensures access to highquality pregnancy planning and prevention services integrated with other preventive health care for women men and adolescents with priority given to lowincome and uninsured individuals who may otherwise lack access regardless of income and health insurance status.
TMFC is credentialed as a Community Provider in the VA Community Care Network (CCN). The CCN is the VAs direct link with community providers to ensure Veterans receive timely highquality care.

TriMed Family Cares fullservice walkin/drivethru pharmacy is conveniently located at the rear of our parking lot. Our onsite pharmacy participates in the 340B Drug Discount Program a federal government program requiring drug manufacturers to provide outpatient at significantly reduced prices. These prescription drug price savings are directly passed onto you as an TMFC patient regardless of insurance status.
Our pharmacy is open to the generalpublic and offers over the counter products free selfblood pressure monitoring hasslefree prescription transfer and medication counseling and therapy management. We accept Medicare Medicaid and most private insurances and offer sliding fee scale pricing to patients who qualify.
