We are currently seeking a Physician Assistant to manage patients in skilled nursing and longterm care facilities. We support a balanced work life with job fulfillment for all our employees. The incoming provider will commit to the highest quality of care using an organized multidisciplinary team approach to provide continuity of care throughout the postacute continuum. Our patient population served includes adults and geriatrics.
Responsibilities
- Embrace the philosophy of our company to render care in a manner approach and with the skill that promotes maintenance or enhancement of each patients clinical status and quality of life.
- Conduct Annual Wellness Visits on all longterm Nursing Facility residents and may perform Annual History and Physicals as requested by Facilities.
- Conduct clinical interventional and episodic direct care visits and appropriate reevaluation of care visits.
- Provide ongoing evaluation and treatment of chronic comorbid illnesses as medically appropriate
- Evaluate/treat residents for changes in condition.
- Review medications orders and diagnostic tests and respond to clinical abnormalities appropriately for all patients under their care as allowed and/or requested by collaborating physicians.
- Coordinate care with collaborating physicians as indicated for overall medical management.
- Assist with facilitywide illnesses where appropriate collaborations exist.
- Coordinate care with other disciplines such as Therapists Specialists and Social Workers.
- Maintain communication with families/attend family conferences when possible.
- Other duties as assigned
Requirements
- Valid MN Nurse Practitioner License
- Masters Degree in Nursing from accredited Colleges or Universities.
- Experience in nursing is required and some experience as a Nurse Practitioner/Provider is preferred but not necessary
- Demonstrated nursing/NP critical thinking skills applicable to the aged chronically ill and convalescent resident.
- Strong leadership interpersonal and problemsolving skills preferred.
- Excellent written and oral communication skills
- Possess traits of punctuality organizational skills and ability to function within a team.
- DEA License
- Proof of COVID Vaccination
