What you will be doing: Care Management Manager (MSW or RN)
- Lead and manage the Care Management Team of RNs and Social Workers.
- Oversee the daily operations and clinical practice of the assigned service areas.
- Supervise Case Managers and Care Management Coordinator support staff.
- Ensure proper case management of patients through the continuum of care.
- Advocate for and assist patients in achieving optimal health and access to care.
- Guide the appropriate utilization of resources.
Experience you will need: Care Management Manager (MSW or RN)
- Bachelor’s degree in nursing or masters degree in social work.
- 3 years of experience in an ambulatory or acute care setting.
- 1 to 3 years of management experience in a healthcare setting.
- Strong project management and operations experience.
- Masters degree in nursing or health administration (preferred).
- Case Management Certification (CCM or ACM) upon hire (preferred).
