JOB SUMMARY:

In conjunction with the Head Nurse provides first line planning coordinating coaching and organizing of daily operations to ensure quality patient care in the assigned practice group. Responsible for providing direct nursing care to Health Center patients as needed based on patient demand and staff capacity. Functions under the supervision of the Head Nurse accepts guidance from the Associate Medical Director (AMD) Physicians Nurse Practitioners and/or Physicians Assistants. Acts as a resource and problem solver within the practice group; is responsible for effective clinical team building.

JOB DUTIES AND RESPONSIBILITIES:

  • In conjunction with the head nurse plans directs coaches problem solves and supervises the day to day work of the clinical support staff.
  • Troubleshoots staff and patient questions and problems as necessary; documents and communicates issues and resolutions to the Head Nurse on a regular basis.
  • Works closely with Head Nurse to assess systems and suggest improvements in systems and processes.
  • Maintains a cooperative relationship with all health care providers Operations Manager (OM) and Business Office Supervisor (BOS) to determine appropriate patient appointments and scheduling needs.
  • Provides feedback and input to Director of Patient Services (DPS) and Head Nurse (HN) on performance and training needs of support staff.
  • Monitors patient and clinical support staff scheduling ensuring adequate services and staff coverage at all times. Monitors patient flow and in the case of patient overflow assists in preparation of patients for exam including taking of vital signs and presenting problems. Performs related work as assigned and required to meet program needs; fills in for clinical positions as needed in the planned and unplanned absence of clinical support staff.
  • Leads daily clinical staff huddles and participates in routine clinical and monthly total staff meetings; meets routinely with HN to ensure that coordination and effective communication prevails.
  • Assists provider with examinations diagnostic procedures and treatments including height weight blood draws etc.
  • Administers and documents immunizations allergy shots and other injections as prescribed by provider.
  • Documents all activities/interventions patient responses medication dispensed and/or prescribed etc. in the medical record paper and electronic.
  • Assists with prescription line and referrals for specialty care as needed; arranges for coverage when staff is on planned or unplanned leave.
  • Consults with providers for prescription refills test results etc.
  • Promotes wellness by providing patient education materials and communicating provider instructions and advice.
  • Adheres to infection control safety guidelines and confidentiality policies.
  • Ensures that agency and program orientation for new clinical staff is conducted timely and according to established policies: ensures adequate onsite training is provided for applicable staff.
  • Provides information on community agencies to patients when appropriate.
  • Oversees and monitors clinical supply ordering and inventory.
  • Ensures internal inventory controls are followed for use and storage of pharmaceuticals and other medical supplies.
  • Participates in departments Continuous Quality Improvement (CQI) functions and assists with identifying and resolving clinical problems; ensures all clinical staff is active participants in quality improvement process; ensure compliance with standards laws regulations and procedures.
  • Attends professional conferences and seminars for professional development knowledge and skill enhancement as assigned.

JOB REQUIREMENTS:

  • Must have advanced knowledge in multidisciplinary practice groups and ambulatory care nursing principles and protocols.
  • Must have knowledge of policies and procedures related to quality improvement infection control environmental safety and patient confidentiality.
  • Must have knowledge of medical chart reporting/recording and filing.
  • Must be familiar with program/agency/regulatory policies and procedures.
  • Must have the ability to work with a large cross section of staff and patients in a warm nonjudgmental objective manner.
  • Must maintain a high degree of confidentiality; and exhibit professionalism good judgment and flexibility.
  • Must have the ability to work well with others as a team and be a positive role model to other staff and patients.
  • Must demonstrate effective leadership skills & team building; be able to assist in directing and monitoring the work of others.
  • Must have knowledge of public services and health care related resources.
  • Must be flexible and able to respond to ever changing demands. Must be flexible with work hours and have the ability to work with minimal supervision.
  • Must have acceptable physical and mental health to carry out the responsibilities of the position including standing for long periods of time physically assisting patients and balancing stress in a healthy manner.
  • Must pass annual competency testing attend required inservices and seminars and maintain current LPN license.
  • Must maintain current basic life support certification; must become HIV counseling certified where applicable.
  • Travel to other agency locations may be required.
  • Must possess a valid New York State Drivers License with a good driving record and have reliable transportation

MINIMUM QUALIFICATIONS:

  • Must possess current New York State Practical Nurses License
  • 5 7 years experience as an LPN a portion of which must be in ambulatory care; supervisory experience preferred
