STRATEGIC FUNCTION Provide comprehensive case management services that will support families to foster their childs development and assist families to attain their personal goals maintain appropriate files and complete necessary reports. Conducts home visits using Parents as Teachers curriculum. Conduct home visits to young children and their families utilizing a research based (PAT) curriculum biweekly or monthly / 60minutes per visit. Complete documentation of home visits group connection experiences and other relevant activities. DUTIES/RESPONSIBILITIESMay include the following: - Develop and maintain positive open and productive team partnership with families community partners and staff while maintaining professional boundaries.
- Adhere to the Social Work Code of Ethics.
- Relying upon the familys strengths work with the family to assess needs and assure that all interventions promote family empowerment selfsufficiency and selfadvocacy.
- In partnership with families develop a realistic short term and longterm goals and the appropriate strategies to achieve these goals.
- Promote family literacy to children ages 05 on the family home.
- Make referrals to needed resources and assist families in accessing resources as necessary and appropriate.
- While following all Transportation Procedures; assist with transporting families to appointments as needed.
- Advocate for families and the Home Visiting Program in the community.
- Ensure families comply with the Home Visiting Program Participation and Consent form including attendance home visits and group connections.
- Obtain and maintain Parents as Teachers (PAT) certification at the agencys expense and provide parents with the opportunity to receive biweekly or monthly home visits to implement PAT curriculum as required.
- Continually update Community Resource information.
- Meet monthly with the Home Visiting Program Coordinator to assign priorities to families on your case load (more frequently as needed).
- Maintain accurate documentation on families by funder and Home Visiting model standards.
- Inform Parents as Teachers Coordinator of family emergencies in a timely manner.
- Report all suspected child abuse to the Child Abuse Hotline atand to the Parents as Teachers Coordinator.
- Assist in gathering recruitment data throughout the year.
- Be active in ongoing self and program assessment in order to participate in continuous improvement of your own skills and program enhancement.
- Promote parental involvement in curriculum planning and overall program activities.
- Conduct screening and ongoing assessment of young childrens development and family wellbeing.
- Flexibility with work schedule and location as needed based on program needs.
In order to promote the school readiness health and wellbeing of the children the Parent Educator will: - Facilitate group connection experiences and other family group activities once per month; this may include planning and assisting with field trips and arranging for special visitors or events. (Follow agency policies and procedures concerning transportation if appropriate.

REQUIREMENT/QUALIFICATIONS Preferred: - Bachelors Degree in Social Work
- Two years experience working with families in their homes
- Experience providing adult and prek children family literacy services
- Bilingual (Spanish and English)
Required: - Bachelors Degree in Social Work Psychology Counseling Substance Abuse Sociology Child Development or related field
- Minimum of oneyear experience working with families
- OR
- Two years implementing a Home Visiting Model
Physical Requirements: - Be able to lift 2530 pounds.
Other Requirements: - Daily access to transportation.
- Valid Texas Drivers LicenseClass C.
- Vehicle insurance (personal injury and liability).
- Must be flexible with hours.
- Criminal Background check required for all staff at the agencys expense.
- Must be at least 21 years old.
