Job Description
.We are looking for a compassionate and detailoriented Care Manager to oversee client care plans and ensure highquality services for individuals in need. The ideal candidate will have strong organizational and communication skills to collaborate with caregivers clients and families to optimize care solutions.
Key Responsibilities:
- Develop and oversee individualized care plans for clients.
- Coordinate with caregivers healthcare professionals and families.
- Monitor client progress and adjust care plans as needed.
- Ensure compliance with homecare policies and procedures.
- Advocate for clients needs and provide emotional support.
- Maintain accurate documentation and case records.
Qualifications:
- BA or MA in Social Work Healthcare Administration or a related field.
- Previous experience in care management social work or homecare preferred.
- Excellent organizational and problemsolving skills.
- Strong communication and interpersonal abilities.
- Ability to multitask and work in a fastpaced environment.
Salary: $60K$70K
