Garten has an excellent opportunity to join our compassionate and hardworking team as a Daybreak Program Specialist . General Position Summary:

Performs file management; administers medications and maintains documentation; conducts intakes/exits/participates in IDTs of persons individuals with disabilities; writes ISPs develops vocational plans and participates in actual training to individuals with disabilities to ensure the completion of their identified ISP goals; ensures compliance with policies and regulations. Acts as liaison to funding sources residential providers families and employers to coordinate case management services.

Essential Functions/Major Responsibilities:

  • Ensures all record keeping for caseload is current and accurate
  • Performs Intakes and Exits
  • Acts as liaison to funding sources service providers families and other supports
  • Assists Employment Specialist with:
    • Assessing skills capabilities and interests of individuals with disabilities
    • Screening candidates for good job fit
    • Making referrals of qualified job candidates to businesses
    • Onthejob training
  • Participates in providing training to individuals with disabilities to ensure completion of their identified goals
  • Responds in timely manner to support individual onthejob needs
  • Maintains effective communication with all Employment Services staff and employers
  • Ensures medications are administered properly and that medical administration record keeping (MARS) is maintained
  • Advises managers and production staff of applicable Oregon Administrative Rules (OARs)
  • Attends annual ISPs and participates in IDTs
  • Ensures confidentiality of individuals receiving services from Garten
  • Ensures time studies are conducted on a semiannual basis and applicable records updated
  • Ensures documentation of productivity
  • Ensures documentation of tracking ISP goals
  • Ensures compliance with Garten policies and safety and security standards

Specific Job Skills Required:

  • Excellent verbal and written communication skills
  • Excellent customer service negotiation and interpersonal skills with sign language skills desirable
  • Excellent organizational and multitasking skills
  • Ability to maintain accurate recordkeeping/documentation and generate reports
  • General knowledge of training techniques
  • Ability to read write speak and understand English
  • Skills in judgment decisionmaking and balancing multiple priorities
  • Ability to actively participate in team development and teamoriented processes
  • Physical and mental ability to operate computer hardware/software
  • Valid drivers license acceptable driving record and proof of current vehicle insurance
  • Physical ability to stand walk twist bend kneel grasp handle reach kneel occasionally during normal workday
  • Physical ability to occasionally push/pull/lift/carry up to 40 pounds
