About the role
- You will be responsible for assessing and care planning of adults who have need for care and support arising from mental health & OR physical disability.
- Working collaboratively with health partners from (BEH) Barnet Enfield and Haringey mental health trust as well as other health and social care colleagues as part of multiagency approach.
- Responsible for undertaking a range of statutory care and support functions under the Care Act and assessing eligible mental health needs Section 117 aftercare under the Mental Health Act 1983.
- Being part of a supportive and coproductive team under a united & vibrant service area.
- Conducting personcentred and strengthsbased assessments to meet identified social care needs.
- To plan relevant responses to meet individual needs in collaboration with relatives carers advocates and representatives and in consultation with health and other agencies.
- Responsible for safeguarding adults at risk which is a fundamental element implementing principles embedded in all procedures practices professional advice decisions and support.
- Conducting individual complex assessments and safeguarding enquiries to establish clear relevant proportionate and appropriate options for the individual to meet their identified outcomes.
About you
- Hold relevant Social Work degree qualification and registered as a Social Worker with the Social Work England.
- Understanding and experience in working with vulnerable adults.
- Relevant experience in operating effectively as a practising Social Worker.
- Understanding and experience in operating with a comprehensive assessment framework including selfassessment.
- Experience in preparing clear and accurate correspondence reports and other written documentation.
- Record of high performance in successfully delivering a customerfocused service to demanding targets and objectives.
- Experience in actively and effectively participating in crossfunctional networks and local groups.
