Job Summary:
The Blood and Marrow Nurse Clinician operates under a clinical case management model to provide and coordinate care to patients during all phases of the treatment and transplant process in the BMT Program. In collaboration with physicians residents/fellows patients and families the Nurse Clinician (NC) gathers data and uses his/her knowledge of complex health problems to provide and facilitate the delivery of quality evidencedbased care while promoting efficient and effective patient outcomes in both the inpatient and outpatient setting. This position provides education and support to physicians staff patients and families regarding innovations/evidence for quality outcomes. In collaboration with physicians staff patient and families the NC facilitates implementation of the plan of care during all phases of treatment and facilitates communication among patients/family and all care providers. The NC takes primary responsibility for admission and discharge planning. The NC is cognizant of the psychosocial and financial impact that disease and hospitalization has on the patient and family and acts as an advocate to identify and obtain needed resources and support. Additionally the NC assists the physician with procedures (including bone marrow harvests and administration of stem cell products) triages phone calls rotates in 24 hour oncall responsibilities and participates in clinical research. The BMT Nurse Clinicians use Nursing Practice Competencies to achieve safe optimal patient and family centered care health outcomes; Leadership Competencies to coordinate patient care and communicate with the Care Team; Professional Profile Competencies to develop professionally as a lifelong learner and Practice Outcomes Competencies to evaluate care based on evidence the ULH quality model and patient safety goals.
