Know Your Rights: the supervision of the Director of Pharmacy and clinical coordinator (if applicable) coordinates the activities of the pharmacy department to provide the highest quality of pharmaceutical care to all patients served by the hospital while ensuring patient safety. Conducts 50 or more of professional time in direct patient care activities including but not limited to interdisciplinary rounding patient counseling or participation in patient care committees. Provision of direct patient care encompasses activities such as the direct observation and evaluation of the patient and his/her medicationrelated needs; the initiation modification or discontinuation of patientspecific pharmacotherapy; and the ongoing pharmacotherapeutic monitoring and followup of patients in collaboration with other health professionals. The scope of patient care provided may include: General Medicine/Surgical Intensive Care Services Obstetrics & Gynecology Pediatrics Emergency Care Oncology Perioperative Geriatrics and Outpatient Services.
