Vancouver Coastal Health is looking for a Community Mental Health Clinician (Case Management)to join the Raven Song Mental Health team in Vancouver BC. Apply today to join our team!

The Vancouver Community Adult Mental Health and Substance Use (MHSU) team supports adults 19 years of age and older with moderate to severe mental health substance use and concurrent mental health and substance use diorders or conditions. This program is designed to be neighbourhoodbased and works in an interdisciplinary team to encourage collaboration and partnerships with local community serviecs including family physicians and other care providers.

As a Community Mental Health Clinician you will:

  • Provide client care to the seriously mentally ill adult population and who are at risk due to a combination of psychiatric symptoms complex medical/social factors or significant caregiver stress.
  • Collaborate with interdisciplinary team members educational resources and other health care professionals/providers to achieve excellence in health services and education.

Qualifications

Education & Experience

  • Masters Degree in Health Science discipline from a recognized program.
  • Three 3 years recent related experience working with individuals with severe and persistent psychiatric illness or an equivalent combination of education training and experience.
  • Valid B.C. drivers license. Local area travel requires the use of a personal vehicle for which mileage reimbursement is paid.

Knowledge Skills & Abilities

  • Knowledge of the principles of recovery in mental illness.
  • Working knowledge of psychiatric theory and practice within a client centered approach to the adult population.
  • Sound knowledge of crisis intervention and community resources.
  • Demonstrated knowledge of psychiatric assessment skills.
  • Demonstrated knowledge and skills to provide case coordination.
  • Demonstrated knowledge of counseling skills.
  • Demonstrated ability to communicate effectively both verbally and in the writing of reports correspondence and other documentation.
  • Demonstrated ability to independently plan organize and prioritize workload and adapt to a changing workload.
  • Comprehensive knowledge of other health care disciplines and their role in patient/client care.
  • Demonstrated ability to recognize when referral to another health care professional.
  • General of knowledge Trauma Informed Practice and Harm Reduction approaches and Strengthsbased care.
  • Demonstrated ability to promote Indigenous Cultural Safety.
  • Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.
  • Sound interpersonal skills
  • Ability to work effectively in an interdisciplinary team environment.
  • Ability to prepare case summaries and clinical records.
  • Basic computer literacy.
  • Physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.

As per Ministry of Health policy all health care workers working in publiclyfunded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health you will be asked to provide this information as part of the onboarding process.
