We are seeking a compassionate and detailoriented Case Worker to join our team. The successful candidate will play a crucial role in assessing client needs developing care plans and ensuring the effective delivery of home care services. This role requires excellent communication problemsolving and organizational skills to support both clients and care staff.

Key Responsibilities:

  • Client Assessments: Conduct initial and ongoing assessments of clients to determine their care needs preferences and risks.
  • Care Planning: Develop implement and review individualized care plans in collaboration with clients families and care teams.
  • Liaison & Advocacy: Act as a key point of contact for clients families care workers and external professionals (e.g. social workers healthcare teams).
  • Compliance & Safeguarding: Ensure that all care services align with CQC regulations company policies and safeguarding protocols.
  • Monitoring & Quality Assurance: Regularly review care plans conduct visits and gather feedback to ensure the highest standards of care.
  • Staff Support & Training: Provide guidance and support to care workers ensuring they understand and follow client care plans effectively.
  • Crisis & Risk Management: Address any urgent client concerns respond to safeguarding issues and report incidents in a timely manner.
  • Administrative Duties: Maintain accurate records update client files and ensure documentation is completed in compliance with regulatory requirements.


Requirements

Previous experience in a case worker care coordinator or similar role within the health and social care sector.

Minimum Level 3 in Health and Social Care

Strong understanding of domiciliary care personcentered planning and safeguarding principles.

Excellent communication and interpersonal skills.

Strong problemsolving and decisionmaking abilities.

Knowledge of CQC standards and compliance requirements.

Ability to work independently and collaboratively within a team.

Full UK driving license and access to a vehicle (preferred).

NVQ Level 3 or higher in Health & Social Care (desirable).
