Qualifications
Education: Graduate of a Social Work accredited program (BSW or MSW)
Licensure: Licensed as a Social Worker in the State of Minnesota
Experience: Minimum of two years of experience as a social worker strong preference for previous hospice or palliative care experience.
Skills: Ability to work independently and seek consultation and support as appropriate skilled in crisis intervention and response experience with communitybased resources and referral process advance care planning facilitation familiarity with grief support acceptance of different social racial cultural and religious backgrounds flexibility innovation and strong interpersonal skills ability to cope or seek support for workrelated mental and emotional stress and ability to value all members of the interdisciplinary team.
Transportation: must have a current valid drivers license with auto insurance and reliable transportation.
Position Details
- MondayFriday with compensated oncall rotation schedule.
