Location:Skilled Nursing Facilities WI Rapids/Stevens Point areas
FullTime: Monday Friday No nightweekendor holidays coverage

Position Overview:Oak Medical is seeking an experienced Nurse Practitioner withexpertisein internal medicine to deliver highquality onsite medical care in skilled nursing facilities. This role is focused on providing both acute and chronic care management for adult and geriatric patients. The APP will work collaboratively withsupervising physiciansto develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.
Key Responsibilities:
- Deliver primary care for adult and geriatric patients addressing both acute and chronic health conditions.
- Develop personalized treatment plans in close coordination withthefacilitycare teamand other healthcare providers.
- Establish medical diagnosis prescribe medications perform diagnostic and therapeutic procedures and other care coordination as needed.
- Ensuretimelyaccurate and thorough documentation of patientassessmentand care plans.
- Work closely with specialty providers (e.g. wound care psychiatry) to coordinate integrated care when needed.
- Communicate changes in patient conditions and treatment plans to the clinical team.
- Communicate withpatientsfamiliesand stafftosupport understanding of care plans and interventions.
- Foster strong relationships with patients their families and the facility staff to enhance care quality.
Qualifications:
- Advanced Practice Provider license (NP or PA).
- Valid Wisconsin APNP or PAC License.
- Valid DEA.
- AHA BLS certified.
- Minimum 1yearAdvance Practice Providerexperience (preferred.
- Experience working in skilled nursing facilities or similar settings is preferred.
- Strong diagnostic clinical and interpersonal skills.
- Ability to work independently and collaborate with interdisciplinary teams.
- Commitment to providing compassionate patientcentered care.
