Job Description
TheWound/Ostomy Nurse Clinician assumes roles of: clinician consultant changeagent educator and participates in research activities. He/she providesexpert clinical consultation and education to nurses family patient HCTregarding their responsibilities for coordinating all aspects of managing anddelivering quality cost effective patient and family centered carespecifically in the areas of wound ostomy and continence. The Wound/OstomyNurse Clinician collaborates with the nurse and physician to assure that allpatients have a comprehensive plan of care that provides continuity across allsettings. The Wound/Ostomy Nurse Clinician acts as a resource to the nurse inproviding patient/family education. The Wound/Ostomy Nurse Clinician evaluatesthe success of care coordination and monitors the quality and continuity ofcare formally and informally through data analysis direct observation chartreview one to one interaction and/or collaboration with the health care teamAdministrators Multidisciplinary Practice or Manager.
