Job Description
Empath Hospice a member of Empath Health is currently seeking a dedicated community outreach Registered Nurse (RN) to join our team. The ideal candidate will be passionate about advocating for individuals and families within the community providing support resources and services to enhance their wellbeing.
The Care Navigation Mobile Outreach Registered Nurse (RN) is responsible for coordinating service inquiries as well as offering prompt access within the Empath Health Network of Care for targeted communities via a community outreach vehicle. Maintains excellence in customer service by being caring compassionate empathetic and responsive to the needs of all who reach out to us. Serve as a valuable participant on committees within our community hospitals. Embracing the Empath mission vision and values while advocating for optimal access to the right care at the right time in any care setting.
Position Requirements:
- Education and/or Experience: Associates degree (A.A. or equivalent from twoyear college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
- Licensure:
- Valid Florida license as a Registered Nurse
- Valid Florida State Drivers License
- Continuing Education: As required for licensure.
- Certifications: Valid CPR certification from the American Red Cross or American Heart Association.
- Bilingual fluent in English and Spanish
- Basic knowledge of nonhospice related healthcare and community resources.
- For fieldbased positions employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance a copy of which will be placed in the employees HR file.
- Versed in Diversity Equity and Inclusion
- Preferred Qualification: 1 year experience working in migrant communities and underserved populations and demographics
- Preferred Qualification: Experience maneuvering and operating a mobile unit. Enhanced DMV licensure not required.
