Job Duties/Responsibilities

Direct Care & Coordination of Services

  1. Attend to individuals daily needs as well as foster increased independence and selfdetermination.
  2. Teach socialization skills and activities of daily living including: household maintenance; selfcare and hygiene; selfawareness and human development menu planning food shopping and preparation; money management; transportation and community orientation; and utilization of community resources.
  3. Follow the Department of Public Healths (DPH) Medication Administration Program (MAP) policies which include but are not limited to training requirements medication administration medication security and corresponding documentation including in BAMSIs Electronic Health Care Record.
  4. Ensure individuals nutritional needs are met based on personal preferences and according to prescribed diet. Provide support with preparation and eating as necessary.
  5. Promote physical health and wellness routines.
  6. Follow individualized protocols for medical and safety needs to ensure proper steps for prevention maintenance follow up and reporting take place as required.
  7. Use supportive and adaptive healthrelated protections/equipment as prescribed. Assist individuals as needed and required with cleaning of equipment as directed.
  8. Support individuals served with achieving the goals and objectives outlined in service/support plans.
  9. Use approved crisis management techniques to deescalate individuals and prevent injury.

Positive Behavior Supports & Community Connections

  1. Utilize Positive Behavior Supports (PBS) practices to create a thoughtful and supportive environment by ensuring activities plans and routines reflect the preferences needs and goals of individuals.
  2. Use integrated strategies to support personal strengths and assets of each individual served.
  3. Support individuals to make decisions across all environments to the greatest level of each persons ability.
  4. Role model behavioral awareness through demonstrating and encouraging prosocial behaviors and maintaining appropriate professional boundaries with individuals served.
  5. Learn and utilize communication tools which include but are not limited to email tablets and letter boards taking into consideration the method most suited to individual needs. Communicate in the preferred method and language of those you provide support.
  6. Promote healthy living and relationship building with all people supported through individualized social spiritual and recreational opportunities offered in membership access and integration in their communities.
  7. Assist with access to community including transportation as needed to individuals served to access community. Examples include but are not limited to transporting individuals served to and from day programs work medical appointments to family and friends visits and other recreation and leisure activities as assigned.

Communication Documentation & Teamwork

  1. Ensure person served safety and compliance with program standards for staff coverage and reportable incidents including assistance to the administrator on call whenever possible as assigned to provide support/coverage needs to other sites or locations as identified and required for immediate needs of person served.
  2. Follow funder and agency protocol for reporting significant incidents and alleged abuse and/or neglect while fulfilling legal responsibility as a mandated reporter.
  3. Work with community physicians and staff to develop and implement effective medical treatment plans.
  4. Assist with admissions and discharges to and from hospitals and rehabilitation centers.
  5. Assess document and appropriately utilize necessary data daily progress notes and staff logs and complete all other an program required documentation including BAMSIs Electronic Health Care Record.
  6. Notify Program Manager and staff of any medical problems that may hinder an individuals progress or that may be aggravated by activities at the program.
  7. Review medical records and physician orders to ensure program compliance updating information as needed.
  8. Maintain knowledge of ongoing program issues and developments as well as informing supervisor of program issues and concerns.
  9. Attend and participate in scheduled staff meetings.
  10. Communicate in a professional and respectful manner.
  11. Work in other residential programs when needed. May work evenings weekends and holidays as needed.
  12. Assist with training and orienting employees new to the program.
  13. Perform duties to reflect agency policies and procedures.
  14. Treat all individuals employees and visitors with respect and dignity.
  15. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
  16. Float to other programs to provide support when needed.
  17. Attend staff meetings complete all required trainings and follow all funder state/federal and BAMSI policy.

Job Requirements

To perform this position successfully you must be able to perform each essential duty and responsibility satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required.

  • Knowledge of the theories methods procedures and practices of nursing care and physical rehabilitation as applicable to community residential services.
  • Two years experience working with individuals with acquired brain injury or similar disabilities preferred.
  • Demonstrates proficiency in English both in writing and verbally.
  • Demonstrates ability to conduct review and interpret assessments and medical treatments.
  • Ability to practice safe driving.
  • Interpersonal skills.
  • Ability to work within Microsoft Office products.

Education/Licensure Requirements

  • Massachusetts Nursing License (LPN) required.
  • Must possess a valid drivers license in state of residence a satisfactory driving record and at least 3 years of driving experience. Ability to show proof of valid drivers license upon hire and thereafter as requested. Employees must follow the standards set by BAMSI for good driving as outlined in the Safe Driverpolicy and must remain in compliance with the safe driving training.
  • In instances where employees are asked to use their own vehicle to drive/transport individuals served the appropriate automobile insurance coverage on their vehicle will be required to be shown upon hire and annually maintained thereafter. In the event of an accident while operating the employees own vehicle the employees automobile insurance would remain the primary insurance and BAMSI insurance would be secondary. Employee vehicles must also be in good working order and have valid documentation including a valid inspection sticker and registration.

  • Ability to lift push pull and maneuver up to 25 pounds 75 pounds. Lifting requirements will increase as the complexity of the residential home increases and depending on the specific needs of person served.
  • Ability to reach bend stand sit and walk.
  • Repetitive and substantial movements of the wrists hands and/or fingers.
  • The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
  • Work effectively in a moderate to occasionally loud work environment.
