SWAMS delivers a wide range of community programs and has a strong growth strategy through partnership opportunities and future community development.
SWAMS is currently seeking to recruit a Midwives to join their dynamic Child & Maternal Health Team in either a casual or permanent capacity. The Midwife will provide comprehensive antenatal and postnatal care to the Aboriginal women of the greater Bunbury Region. A key component of the role is to work in partnership with other maternity providers to ensure women receive timely and culturally appropriate care with the aim of closing the gap in maternal health outcomes.
The role of the Midwife working within a multidisciplinary team incorporates the following activities: unsupervised clinical practice coordination of care health promotion patient advocacy collaborative patient management research communication and accurate and timely documentation. SWAMS staff strive to implement best practice Primary Health care to the people of the South West and to coordinate multidisciplinary care planning internal and external referral including referral for specialist care and allied health. This is done through coordination of the patient journey both in the clinic and the community.
