Scope of Position: Receives general direction from the Medical Director or designee and the chief technologist. The Polysomnography Specialist performs standard and complex polysomnograms. Provides inserve instruction and supervision to trainees or sleep technologists and may participate in research projects. Required to work independently and make appropriate decisions regarding the diagnosis and treatment of benign to the most complex patients. May perform clerical administrative and other laboratory duties as required.Principal Duties and Responsibilities: Examines physicians orders and protocols for each sleep study. Reviews existing information in patients record and recognizes chief sleep/wake complaint.Follows the type of recording to be administered based on the medical directors /designated on call MDs orders. Interviews patients to assess current medications recent caffeine/alcohol ingestion and current sleep pattern and the need for treatment intervention during the study. Communicates with ordering physician or Unit Director to clarify unclear or inappropriate orders and communicates information regarding patients status. Must spend appropriate time in servicing the patient regarding the possible use of CPAP/BIPAP equipment. The patients fears and apprehension concerning CPAP should be reduced or eliminated by the inservice. Works with the patient throughout the night to insure patient compliance to CPAP/BiLevel. Sets up/calibrates monitoring equipment with appropriate montage. Measures and applies necessary EEG electrodes using International 10/20 System. Applies appropriate EKG EMG and EOG monitoring electrodes. Places and securely attaches transduce4rs to measure nasal oral airflow respiratory effort and oximetry. Administers supplemental oxygen under physicians order. Performs physiological calibrations verifies the integrity of the recording. Institutes lights out and notes patient data every 20 minutes on the Grass log sheet. Monitors the polysomnogram for clinical and physiological events. Must be able to interpret severely abnormal data/EKG rhythms and report such findings to sleep physician.Makes sure that the record is identified for immediate scoring and interpretation. Notes and corrects equipment malfunctions.Understand indications/contraindications and adverse effects of nasal continuous positive airway pressure (NCPAP BIPAP and oxygen administration) applies and titrates therapy based on physicians orders and established protocols. Recognizes and responds appropriately to patients medical problems and physiological events especially cardiac arrhythmias respiratory events and seizures. Alters the patients environment or changes the patients body position as needed to obtain the required sleep/awake data. Evaluates routine or abnormal EEG when it applies. Carefully wakes the patient and records posttest calibrations. Removes the electrodes and transducers and assists the patient with hygiene as necessary. Complies with Standard Precautions and Infection Control procedures. Properly disinfects and sterilizes the equipment the electrodes and the transducers and monitors use during the study. Prepares a detailed report of the data and intervention used to treat the patient. As needed scores the polysomnograms and generates a report summarizing sleep/wake periods REM episodes arousal analysis EEG phenomena cardiac events respiratory events limb movements effects of CPAP BIPAP and supplemental oxygen and other physiological events according to current guidelines. Expected to score whenever time permits. As needed conducts and scores multiple sleep latency tests using standard guidelines.As needed prepares a report of the MSLT/MWT for the interpreting M.D. As needed follows the lab protocols regarding set up and take down of a MSLT/MWT patient. Attends seminars and professional meetings to keep knowledge and skills updated. Attends department meetings when required. Must be able to work flexible hours with the primary emphasis on the night shift. Comply with applicable laws regulations guidelines and standards regarding safety and infection control issues. Participate in equipment care and maintenance. Position requirements: Must be registered by the BRPT. Experience is preferred Maintain current CPR or BCLS certification. Demonstrate effective written and spoken communication skills. Demonstrate appropriate social skills. Demonstrate ability to follow direction. Must stay current with the AASM guidelines for acquiring 30 CMEs averaged over a 3 year period. Must have leadership qualities and take on a lead role as needed Comply with applicable laws regulations guidelines and standards regarding safety and infection control issues.
