Job Description Case Manager
Case Manager will have a caseload of around 5 families. Case Manager will ensure safety and stabilization of the families assigned to them. The position is 35 40 hours per week fulltime with evenings and weekends required as needed. The overarching goal of Case Manager is to avoid outofhome placement of youth on their caseload.
Duties Case Manager
- Case Manager will make home visits to family on caseload and assess safety
- Case Manager will provide oncall services to family
- Case Manager will conduct crisis intervention with family as needed
- Case Manager will assist family in creating a plan to improve family functioning
- Case Manager will provide parenting skills counseling stress management anger management and communication skills development as needed
- Case Manager will connect the family with community resources
Requirements Case Manager
- High School Diploma with at least 12 college credits in Human Service/Social Service field
- Associate Degree in Human Service/Social Service field
- Bachelors Degree or Higher in a related field with a valid drivers license and a reliable vehicle
AND
At least 12 years of experience working with children and/or adults with behavioral health/mental health disorders
Job Type: Fulltime
Salary: $20.00 $21.00 per hour
Schedule:
- 8 hour shift
- Weekend availability
- Direct facetoface services
License/Certification:
- Drivers License AND a reliable vehicle (Preferred)
