Job Description

Adult & Child Health is an integrated healthcare provider in Central Indiana. Providing highquality affordable care including primary care behavioral health services substance abuse treatment mobile crisis response communitybased services homeless outreach schoolbased treatment and more.
About this role: As an Integrated Health Nurse is a great opportunity to be able to collaborate with multiple teams to provide the best care possible to our clients.
The Location and hours: A&C is based in Central Indiana. For this position we service the following county: Marion Co.
Hours are MF 75pm or with flexible scheduling available.Some travel between locations
Responsibilities include but not limited to:
- Administer injections and collaborate with BH providers to ensure client is receiving proper doses.
- Efficiently document in EMR.
- Process medication refills.
- Follow up with labs and assist on rooming clients.
- Bachelors preferred.
- Indiana RN license required or LPN
