Job Description

RadiusLicensed Practical Nurses are a vitalcomponentof the Team Based care atRadius. Clinical teams of nurses physicians nurse practitioners medical office assistants outreach workers health advocates mental health therapists and other professionals offer comprehensive wraparound care to Edmontons most marginalized population. Nursing staff are required to work tofullscopeof practice within a primary health care setting to serve all aspects of client needs. LPNs are leaders and independent practitioners atRadius working collaboratively to ensure high quality care continuity of health services and all possible social and medical supports for clients.

This role will be based primarily offsite at the Navigation Centre.Thepurpose of the Navigation Centre is toconnect people living in encampments withanumber ofcrucial services in one location.


Key Responsibilities

  • Work in an interdisciplinary team to provide medical supports ina primary care setting
  • Incollaboration with the healthcare team providing the following healthcare services:
    • Assessment and triage
    • Basic treatment of injuries and ailments(includingwound care)
    • Laboratory collection
    • Medicationmanagement (including medication administration and medicationassistance)
    • Management ofsubstance misuse disorders
    • Continuity of care
    • Client advocacy
  • Be an essential part of clinic workflow including managing patients through all aspects of their interactions atRadius managing appointments and information within the Electronic Medical Record and providing direct and indirect supports through the clinic space

Qualifications

  • Current registration with CLPNA
  • Current CPR
  • Mandatory vulnerable sector check and suitable criminal record check
  • Experience with marginalized populations (e.g.homeless serving sector social service agencies)an asset
  • *applicationsfrom paramedics will also be considered

Knowledge/Skills

  • Able to balance numerous demands and conflicting priorities strong problemsolving skills.
  • Ability to work cooperatively within a multidisciplinary team
  • Ability to demonstrate patience and understanding for those with addictions and behavioral disorders
  • Strong computer proficiency with experience with MedAccess EMR an asset
  • Excellent communication skills.
  • Has a good understanding of traumainformed care and must be able to operate within a harm reduction model of care
  • Excellent understanding of cultural awareness an asset
