Job Description

Pikes Peak Hospice & Palliative Care has an immediate opening for a Manager Psychosocial Services.

Location: Colorado Springs CO

Status: Fulltime

Schedule: MondayFriday 85

Pay Range: $62836 $80329.60 annually

Supplemental Pay:Based on position schedule and/or availability: Overtime

Mileage and Expense Reimbursement: Sixtytwo and a half cents per mile one of the highest in the industry!

REWARDING WORK YOU WILL DO:

Oversees psychosocial team members bereavement services and volunteer services. Positively impacts regulatory compliance quality scores and ADC growth. Collaborates with business development and clinical operations leadership to align on initiatives and programs that will achieve desired results. Compiles and analyzes data and recommends plans for improvement. Ensures continuity of care between direct care and bereavement services while managing the psychosocial team including Social Workers Chaplains Volunteers and other supportive services that may be affiliate specific. Facilitates grief support groups and community outreach programs ensuring the holistic integration of volunteer and psychosocial support in hospice and palliative care. Works closely with other leaders to advance the organizations mission and to elevate brand awareness through community presence and engagement.

  • Manages bereavement services for hospice families ensuring timely completion of assessments and care plans per Medicare Conditions of Participation (COPs).
  • Oversees bereavement programs including community counseling support groups and educational initiatives for team members volunteers and the community. Designs programs that differentiate the organization in the community and add value to the people and referral partners we serve.
  • Ensures compliance with policies HIPAA and DORA regulations to maintain care quality and consistency with hospice and palliative care mission.
  • Coordinates and delivers orientation and ongoing learning and development of chaplains social workers counselors volunteers and volunteer dept. team members.
  • Monitors caseloads productivity and maintains appropriate staffing levels.
  • Cultivates community relationships; develops new bereavement programs based on community needs in alignment with Business Development and Operations.
  • Provides grief counseling and facilitates support groups when needed.
  • Participates in quality improvement programs (QAPI) and ensures team adherence to quality standards like CAHPS and HIS.
  • Oversees patient services by psychosocial team (Social Workers Chaplains Counselors Music Therapists and volunteers) and manages team productivity.
  • Ensures accuracy and timeliness of EMR documentation to reflect patient and family needs.
  • Manages teams workload ensures appropriate patienttostaff ratios and optimizes resource use.
  • Attends and coordinates care meetings (IDT/IDG patient conferences).
  • Resolves patient and client complaints through evaluation and corrective action.
  • Ensures compliance with regulatory agencies participating in surveys and acting as liaison with surveyors.
  • Collaborates with finance to ensure accurate timely billing.
  • Works with Clinical Managers to support teams and ensure compliance with hospice conditions of participation.

WHAT WE ARE GOING TO LOVE ABOUT YOU:

  • Masters Degree in Social Work Counseling Psychology Divinity or related discipline.
  • One to three years of experience as a counselor social worker or chaplain in a medical or mentalhealth setting (preferably home care or hospice); one year management experience.

  • Colorado license in Professional Counseling or Clinical Social Work preferred. Valid Colorado Drivers License and auto insurance.

PHYSICAL REQUIREMENTS:

  • Ability to lift/carry a minimum of 30 lbs
