Job Description


PositionSummary

The Clinical Pharmacistwill work with the Director of Pharmacy Vice President of Clinical Operationslocal care management leadership and staff and local medical leadership toprovide clinical pharmacy services to support the patient centered medicalhome.

Essential Functions

    • Clinical Pharmacy Support to Care Management
    • Complete medication reconciliation andmedication reviews in collaboration with care management staff withinvolvement of members and staff/providers from the hospitals primary carephysician practices and specialist offices as needed
    • Development of membercentered communicationsassessments and recommendations for consideration and followup by appropriateproviders and care management staff.
    • Providedirect consultation and/or follow up with members as needed up to and includingmember home visits when necessary.
    • Documentation of membercentered activities inCCNCs care management documentation platforms
    • Quality Improvement
    • Supervision of Pharmacy Program Assistants
    • Facilitation and troubleshooting of medicationcoverage issues identified for members.
    • Education to Care Management Pharmacies andMedical Providers
    • Perform other duties as required.

Key Activities

    • Serve as a resource for drug informationrequests from care management staff and/or providers.
    • Execute education and training of caremanagement staff on Medication Management policies and procedures andmedication topics.
    • Coordinate and manage the delivery of educationto local care management staff community pharmacists and medical providersabout CCNC and payer pharmacy initiatives.
    • Attend and present at local network meetingsas requested such as Steering Committee Meetings and Medical ManagementMeetings
    • Regular contact with local pharmacists toinclude CPESN pharmacies and pharmacists practicing in local hospitals orpractices as needed to foster relationships with local care management staffand providers.
    • Clearly track and communicate the progress of monthly/quarterlyinitiatives to internal and external stakeholders
    • Assist with medicationrelated qualityimprovement efforts including but not limited to the following areas:
      • Medical Practices
      • Care Management
      • CPESN Pharmacies

Requirements

  • Excellent communication skills both orally and in writing
  • Considerable knowledge of medicationmanagement/pharmaceutical care in the ambulatory environment
  • High level understanding of payer pharmacy programs andpolicies
  • Clinical knowledge and skills to support direct patient care
  • Knowledge of and compliance with federal and stateregulations applicable to the position
  • Analytical skills necessary as independent decisions andproblem solving are required
  • Strong organizational and computer skills required includingvarious office software and Internet

Education and Experience

Registered Pharmacist PharmDpreferred with a minimum of one year of practice experience in any setting

  • Residency training and/or 35years of experience are preferred.
  • Must possess a valid NCPharmacist License.

WorkingConditions

Physical Conditions: The job environment is primarily an office or home environment. Exposure to general office and household conditions as well as communicable diseases could occur. Routinely there may be some minor physical inconveniences or discomfort in the work setting. A moderate amount of traveling/driving is required as well as sitting for moderate periods of time. There is occasional lifting of 2030 pounds necessary to complete a task.

Work Environment: Time will be spent making phone callstransmitting facsimile messages corresponding via email and documentingpatient care activities. Frequent changes are expected on a daily basis;therefore flexibility and adaptability are needed. Evening and weekend workmay be required. Attendance at occasional regional or statewide meetings/training will be required. Occasional member home visits may be required.
