Job Summary:

Under the direction of the Nurse Manager or designee and guided by the policies and procedures of DAP Health the Behavioral Health Licensed Vocational Nurse (LVN) has the clinical responsibility for developing and maintaining a high standard of patient care and daily clinical operations. The Behavioral Health LVN will play a critical role in the patient experience including optimizing clinic flow and working collaboratively with the team to create a patient centered environment.

Job Status: Fulltime Hourly Nonexempt

Duties/Responsibilities:

  • Performs the daytoday operations of the Behavioral Health Clinic as directed by the Nurse Manager or designee.
  • Performs permitted procedures under the direction of the Nurse Manager and/or physicians or designee; assumes clinical responsibility for administration of medications for psychiatric medication managed patients.
  • Ensures patient safety.
  • Gathers patient health maintenance data.
  • Obtains vital signs.
  • Conducts health screens as assigned.
  • Assists with patient flow during clinic hours.
  • Communicates positively with patients and outside parties regarding agency programs; this individual will utilize his/her excellent customer service and communication skills on a daily basis via phone/electronic contact mail correspondence and personal contact with both internal and external customers;
  • Supports the standards of the policies and procedures of the practice and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration and complies with all Ambulatory Services and DAP Health policies and procedures for infection control safety and administrative and clinical practice.
  • Communicates with pharmacies regarding prescription orders preauthorizations of medications and assists with refill requests.
  • Serves as patient liaison.
  • Works closely with Case Management and Home Care staff to ensure continuity of care.
  • Completes all paperwork accurately and efficiently.
  • Requests and retrieves client files as needed.
  • Assists clients with appointments to referral sources.
  • Accesses patientprotected health information as required.
  • Answers telephone and relays messages.
  • Performs other duties as assigned.

Qualifications

Required Skills/Abilities:

  • Proficient in the utilization of the electronic medical record system as it applies to the position.
  • Excellent verbal and written communication skills.
  • Excellent organizational skills and attention to detail.
  • Engagement in complete delivery of quality customer care in an effective and professional manner.
  • Bilingual Spanish/English preferred.

Education and Experience:

  • Active Licensed Vocational Nurse licensure in the State of California.
  • Current American Heart Association or American Red Cross accredited BLS certification.
  • Experience working in behavioral health.
  • Experience working with LGBTQ community.
  • Experience working in FQHC.
  • Experience working with patients with substance use disorders.

Working Conditions and Physical Demands

  • Ability to lift 24 pounds.
  • Operates in a clinical setting at times and requires frequent times of sitting standing repetitive motion and talking.
  • Involves contact with patients or patient specimens. Exposure to blood body fluids nonintact skin or tissue specimens required.
