GENERAL SUMMARY/ OVERVIEW STATEMENT:
Brigham and Womens Primary Care Associates South Huntington is an internal medicine practice designed with the goal to create an environment that maximally fosters healing relationships between patients and their health care team while supporting an outstanding educational experience for future members of that healthcare team. The practice will test innovative models of primary care delivery. It will be continually improving in a way that is accessible to being evaluated and disseminated. The practice will be certifiable at the highest level as a PatientCentered Medical Home.
Position Summary:
We have created a position for a Behavioral Health/Community Resource Support Specialist who will work with Social Workers Nurse Care Coordinators and other primary care staff to ensure that patients receive the resources and services they need. In addition this position is responsible for assisting with the development and continuity of collaborative relationships with community resources for our patients.
In addition the BH/CRSS will be an integral part of the behavioral health team providing support to patients and practices regarding depression management for adults with varying levels of depression with the goal to expand to other behavioral health conditions in the future. S/he will work collaboratively with the team to implement evidencebased services for primary care patients utilizing tools and training from IMPACT Collaborative Care model and Partners HealthCare.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Under the direction of the behavioral health team provide assistance to patients/families around community resources follow up phone calls around medications/questionnaires finding appropriate support systems and supporting Patient as appropriate in their mental health goals.
Under the direction of the primary care team provide additional support services to team identified patients for medical management support: phone support appointment reminders provides support around their health goals.
Provide coordination around transportation needs of patients. This includes though is not limited to: managing Masshealth transportation (PT1 submissions updates) identification of community transport programs provide ongoing transportation scheduling assistance/support to team identified patients.
Provide leadership in the identification of resources and programs that support Patients psychosocial needs. This includes the research and development of materials to effectively communicate said resources to staff and patients and could include: legal financial insurance housing assistance programs health education materials.
Provide advocacy coordination and education of patients/family around appropriate internal and communitybased resources. These include though are not limited to: medication/medical management resources health/wellness programs government programs community agencies housing options inhome support services (elder services meals on wheels day programs and other services as appropriate.
Maintain documentation standards
Maintain identified patient registries; primarily for integrated depression care though this may expand in the future.
Maintain effective communication methods with patients and care team.
Participate as an active member of behavioral health team.
Maintain contact with BWH population management central collaborate in the maintenance of resource database and regularly update staff on new information/resources.
Maintain patient confidentiality in all matters.
Participate in the education of students and residents
Participate in process improvement groups
Early evening hours once a week
Occasional home and/or community visits
Assist coworkers in their duties as requested (within the limitations of training and scope of practice)
Other duties as assigned
QUALIFICATIONS:
Bachelors degree preferably in social work counseling health education psychology or behavioral health
Minimum 2 years in relevant setting (health care and/or behavioral health)
Valid drivers license
Comfort with a patientcentered approach
Aptitude in problemsolving and behavioral change strategies (i.e. Motivational Interviewing and Behavioral Activation)
Ability to serve as a critical liaison to facilitate connections between people
Ability and willingness to work in a timelimited structured and solutionfocused environment
Ability to maintain a professional stance if conflicts arise with other staff
Flexibility to adapt to unforeseen needs or circumstances
Effective use of the Internet and Microsoft Office programs such as Word Excel and Outlook
Demonstrated experience working with diverse and atrisk populations
