Duties:
- Lead the Centers HIV Lead the Centers HIV focused Case Management Programs including both Clinical Case Management and NonClinical Case Management targeting clients with substance use disorders.
- Meet program goals and objectives and those of related grants.
- Supervise program staff within the program and agency by evaluating performance as outlined in the Personnel Policies and job descriptions by providing guidance and support for cases as required by licensing but no less than a biweekly basis. Communicate interpret and enforce program and administrative policies with supervisees at orientation and as new policies are developed. Communicate any staff concerns regarding policies and procedures to the Management Team in the weekly meeting.
- Organize program priorities. Verify client eligibility for program services and track utilization. When new funded activity becomes available convene stakeholders and manage setup and implementation. Conduct periodic audits of client files.
- Exhibit understanding of contract requirements (outcomes training competencies etc. for the program and related grants.
- Carry the supervisors cell phone as scheduled and act as backup for crises in clinical services
- Serve as one of the Centers subject matter experts on HIV care and treatment.
- Conduct outreach and provide presentations to other key stakeholders partner organizations and service providers.
- Conduct psychosocial intake and assessments including a diagnosis according to the current Diagnostic and Statistical Manual of Mental Disorders a diagnostic summary treatment recommendations referrals and preliminary discharge plans. Conduct treatment planning with the participation of the client. Provide psychotherapy and counseling to achieve the objectives and goals in the treatment plan using generally recognized psychotherapy models and theories. Conduct periodic treatment plan reviews with the client. Keep timely and comprehensive progress notes. Conduct discharge planning with the participation of the client and complete a final discharge plan and discharge summary at the time of termination of therapy.
Requirements:
- Graduation from an accredited graduate program in Social Work Psychology Educational Psychology or equivalent degree that provides the capacity for the employee to be licensed as an LCSW or LPC. Four 4 years experience working with clients with at least one 1 year of that experience being with the programs target population(s) of which an internship or field placement may be applied.
- If in recovery no less than four 4 years substance free.
- Access to reliable transportation a current Texas Drivers License and auto insurance.
