DEPARTMENT: SupportedHousingCounselor Jamestown NY Chautauqua County
SCHEDULE: Monday through Friday 9:00 am to 5:00 pm
SALARY: Minimum $19.57/ hr.
AVAILABLE:ASAP
JOB FUNCTION:
Provides case management andsupportservices to consumers. To teach counsel and monitor consumers activities of daily living in asupportedhousingprogram. To establish all necessary linkages to any required services that help consumers gain selfsufficiency become integrated into the community and prevent a consumer from becoming homeless.
MINIMUM QUALIFICATIONS:
Education Experience and Knowledge
OTHER QUALIFICATIONS:
- Have a home telephone.
- Be able to travel to and among various facilities that are multileveled with stairs only.
- Have a valid drivers license and a safe driving record.
- May be required to use personal vehicle for programmatic purposes.
- Criminal Background check approved by NYS Office of Mental Health.
