JOB TITLE: Outreach Therapist
EMPLOYMENT TYPE: Fulltime 40 hours/wk
SCHEDULED HOURS: MondayFriday 9AM1PM start at Danielson Court 15 in Danielson office. NO WEEKENDS or HOLIDAYS.

ABOUT THE PROGRAM: TREATMENT PATHWAYS PROGRAM
To provide a full range of court and community based clinical and community support services for assigned clients with substance use disorders through the entire pretrial process. To facilitate access to appropriate treatment services by providing assessment referral and linkage to community mental health and addictions services. Manages clients care through the clients entire pretrial process.
DUTIES & RESPONSIBILITIES:
- Conducts Clinical and Risk Assessments including: CSSRS psychosocial assessment and level of care assessments
- Collaborates with individuals to create individualized person centered treatment plans and documents clients progress during treatment.
- Provides documentation to court personnel regarding compliance of treatment.
- Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery utilizing stage wise interventions
- Provide psychoeducational rehabilitation services sessions as needed.
- Support individual with linking and engaging with natural (family friends) or community supports
- Develop crisis plans for clients and work with other staff to respond to crisis situations as needed.
- Provide intensive skill building interventions and support for selfmanagement selfadvocacy health and wellbeing relapse prevention and using community resources
- Participate in family education and support activities
- Maintain documentation in compliance with agency and program standards.
- Participates in staff meetings supervision trainings and other meetings as assigned.
- Additional duties as required.
QUALIFICATIONS:
Education: Masters degree in Behavioral Health field or related field.
Experience: Demonstrated indepth knowledge and experience as a psychotherapist working with people with mental illness and/or cooccurring disorders. Demonstrated training and experience in all aspects of mental health treatment assessment evaluation and provision of individual group and family therapy. Relevant experience to program model to which individual is assigned.
Licensure/Certification/Registration: Licenseeligible preferred but not required. Associates level license required.

COMPENSATION*:
$ 60496/00/annually min and up commensurate upon experience.
*Actual rates are determined at the time of offer and are based off relevant experience as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or signon incentives. (Please note for parttime positions this range would be prorated based on hours worked)
