Job Description
Job Title:Nurse Practitioner (NP) Permanent Position
Location: EugeneOR
Employment Type:Permanent
Salary: $125000 to $135000 yearly gross

Job Summary:
As a Nurse Practitioner you will provide primary care diagnostic assessments treatment plans patient education and preventative care to patients. You will collaborate with our multidisciplinary healthcare team to ensure patients receive exceptional care across various specialties and services.
Key Responsibilities:
- Perform comprehensive physical exams and assessments.
- Order interpret and followup on diagnostic tests and laboratory results.
- Diagnose and manage acute and chronic conditions.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Prescribe medications therapies and treatments within the scope of practice.
- Provide patient education and counseling on disease prevention wellness and lifestyle changes.
- Maintain accurate and uptodate patient records in the electronic health record (EHR) system.
Qualifications:
- Masters degree in Nursing from an accredited Nurse Practitioner program.
- Valid Nurse Practitioner license in the state of Oregon.
- Current certification as an Adult Family or Psychiatric Nurse Practitioner (or other appropriate certifications).
- Minimum of 2 years of experience as a Nurse Practitioner preferably in a primary care or specialized setting.
- Current Basic Life Support (BLS) from the American Heart Association (AHA) or the American Red Cross
- COVID Vaccinated
- Recent PPD Test or Chest XRay
- Clear criminal background check(s) & drug screening
- Reliable transportation & smartphone
