Job Description
Director of Case ManagementPosition:
- Salary:$85000$110000
- Schedule:FullTime Direct Hire Day Shift
Director of Case Management Responsibilities:
- Oversee the interdisciplinary plan of care and discharge planning process.
- Ensure services are effective and appropriate with a focus on census management patient care outcomes and key care indicators.
- Lead and mentor the Case Management team to maintain high standards of care and compliance with regulations.
- Collaborate with clinical teams to address patient needs and improve care coordination.
- Monitor utilization review processes to ensure efficient resource use and compliance with payer requirements.
Director of Case ManagementQualifications:
- Experience:At least 3 years of hospitalbased Case Management experience including Utilization Review and Discharge Planning.
- Nursing Candidates:Must hold a bachelors degree in nursing (BSN) with RN licensure.
- Licensure:SD State Licensure required.
- Other Professionals:Must hold a minimum of a bachelors degree (graduate degree preferred).
- Certifications:Current CCM or ACM certification preferred or ability to obtain
